SHPA NEEDS YOUR HELP !!!
SHPA needs your assistance in compiling data that will help drive our SFY2021 Supportive Services Budget request. Below you will find a link to the  
SHPA will use this information to compile a list of supportive housing projects requesting service funding for the next fiscal year and advocate for service funding at the state and federal level.

SHPA is asking for 
New and/or Expansion Projects 
(opened or opening)
SFY2020 July 1, 2019 - June 30 2020 
SFY2021 July 1, 2020 - June 30, 2021

If you are considering an expansion of a current project to which will need service funding please include that as well.

VERY IMPORTANT
Regardless of whether or not you are reporting on a project and if applicable, we are also asking for any funding needs related to the impact of the Minimum Wage increase on your supportive housing projects, specifically those projects funded through IDHS Division of Mental Health and Family and Community Service; Bureau of Homeless Services.
 

Things to consider when completing the survey:

  • Some questions are marked required please enter N/A if the question does not apply to your project(s)
  • Please be sure to respond to the Minimum Wage question, if applicable  (regardless of project info)
  • If you are not the correct staff person to complete this survey we ask that you please forward it to the correct person right away
  • You will be asked how many projects you wish to report, please report on ALL new or expansion projects for the period requested
  • We recommend that you use this pdf version to gather the information prior to completing the electronic survey as you will not be able to stop and start the survey once it is begun.
