Additional support for practices: increasing Hub capacity.
 
In addition to the additional extended access hub capacity announced last week we are pleased to confirm that the CCG has agreed funding for an additional in hours hot hub subject to appropriate premises being identified. This can only happen if practices volunteer suitable space, so please, if you are interested, please take a look at the attached hot hub standards and contact chris.thornton@nhs.net urgently.

The basic criteria for a new in hours hot hub site is:
  • Segregated entrance to reduce patient mixing
  • 2 hot rooms and ideally 2 cold rooms (or 1 with two networked PCs)
  • Availability from 9 August for 8 weeks
  • Availability for clinics 11am to 5pm
Additional nurse capacity has also been secured. We will announce more on this when we’ve confirmed our plans.

We are also looking at whether additional ANP cover in the hubs would help relieve some of the pressure on GP appointments.

Please find attached the updated clinical guidance for the effective use of Hot Hubs. Whilst we are working to expand capacity to meet the current demand for Extended Access services we ask that practices review their procedures for referring patients to the Hot Hubs in light of this guidance to make sure that the service is being used as appropriately as possible.

First Contact Physiotherapy – Extended Access Service

The First Contact Physiotherapy service, available to book as part of the Extended Access Service, is experiencing a high rate of patient DNAs as well as an increasing number of patients attending with unrealistic expectations. 

The intended purpose of this service is to allow patients access to MSK expertise as a first point of contact, often from triage or signposting, in order to free up time in primary care and avoid duplicating work. Patients attending the service will receive a 15 minute appointment during which they will be screened for both MSK and possible non-MSK masqueraders and a plan will be developed for their care. This could be a self-management plan, a referral onto MSK for rehabilitation or further investigations as necessary. 

Although we liaise and link into the wider MSK service we are not actually part of the service. If it is deemed a patient has complex MSK needs then a referral to MSK will be made by our service.

We realise there are huge pressures on the wider MSK service as a result of the COVID-19 pandemic and we are happy to review patients who have been reviewed by a clinician in practice where there is uncertainty as to whether MSK referral is actually required, or to provide some self-management advice as appropriate whilst the patient awaits MSK input. 

We would be grateful if you could please ensure that the patient is involved in a shared decision making process to attend the service to minimise our DNA rates so that both patients and practices get best use of the service.

Covid Vaccination Centre

Darnall Primary Care Centre is holding walk-in vaccination clinics, open to everyone aged 18+. Call in at times below. 1st and 2nd doses (8 weeks between doses).  Walk-ins available, including those not registered with a GP. We also now provide AZ appointments in addition to Pfizer at weekends.
Registering service development interests and expertise

As GP practices you came together to create PCS to do the things that you can't do alone. It's your organisation.

Our experience has shown that to deliver things the way you need them delivered we need to involve clinicians, managers and practice teams from the outset when services are being created.

Rather than wait until we have any specific project in the first stages of development, we would like to be pro-active and build up a register of those with an interest in future project development. So, if you have a specific area you are interested in then let us know so that we can contact you if input is needed in that area.

The PCS team is always keen to hear about anything you would like to raise with us in the way of feedback or any ideas you may have on how we might shape things going forwards. 
 
Dr Andy Hilton and Nicki Doherty keep a lunch time 'drop in' slot available in their diary on Wednesday (Andy) and Friday (Nicki).  If you would like to catch-up with them in one of those, please simply click the button below, and we will get back to you to arrange that.

Please note that these 'drop-in' sessions are in addition to the 1:1 meetings we have been arranging with practices.
