Our 2021-2024 Strategic Plan is now available on our website

In 2021, the South Georgian Bay Community Health Centre (SGBCHC) embarked on the development of a new three-year Strategic Plan to reflect on the organization’s future direction – how the CHC, its clients, community and partners were evolving, and the emerging health care needs within the community. An essential part of this process was engagement with our broader community to help shape our work over the coming years.

The SGBCHC Board of Directors and the management team identified initiatives that support organizational growth and strategic direction within the framework of the CHC's mission, vision and values. Taking into consideration the broader landscape as it pertains to the COVID-19 pandemic and recovery, the planning process included analyzing the CHC's services, programs and initiatives being offered to the community, coupled with the long-term outcomes that the CHC is trying to achieve.

At the heart of this plan is a reaffirmation of the SGBCHC's commitment to addressing the social determinants of health and supporting the most vulnerable living in our communities.

What is self-care and why is it important for your health?

International Self-Care Day is celebrated each year on July 24th. It stresses the importance of self-care as the cornerstone of wellness. On this day, individuals worldwide are encouraged to make self-care a priority in their lives, and a part of their everyday routines.

What is self-care?
The World Health Organization defines self-care as individuals, families and communities promoting and maintaining their own health, preventing disease, and coping with illness and disability, with or without the support of a healthcare worker.

According to this definition, self-care includes everything related to staying physically and mentally healthy — including hygiene, nutrition, exercise, health literacy, and seeking medical care when needed. It’s all the steps you can take to manage stressors in your life, and take care of your own health and well-being.

Why is self-care important to your health?
The benefits of practicing self-care include improved physical, mental, and emotional health and well-being. Research suggests self-care promotes positive health outcomes, such as fostering resilience, living longer, and becoming better equipped to manage stress.

Common examples of self-care include - maintaining a regular sleep routine, eating healthy, spending time in nature, doing a hobby you enjoy, and expressing gratitude. Self-care can look different for everyone, but the behavior of self-care should promote health and happiness for each individual.

Self-care support at the SGBCHC
The South Georgian Bay CHC focuses on the heath and wellbeing of individuals in our community by providing:

Health care services - such as doctors, nurse practitioners, dietitian, system navigators, social workers etc.
Free community programs - such as food workshops, diabetes support groups, mindfulness workshops, chronic disease education and others
Community development - to support and advocate for important community issues such as housing and food insecurity

Our health care services and programs are available to all members of our community at no cost, including those who experience barriers accessing health care.

Are you looking for an opportunity to give back to your community while gaining valuable experience? If so, we want to hear from YOU!

Through our free community programs, our goal is to provide our clients and members of our community with the tools, knowledge and support to achieve optimal health and wellbeing.
We Need Your Talents and Skills!
As a volunteer, you will have the opportunity to learn new skills and join a dynamic team. We value diversity and want our volunteers to share their talents and skills by helping us plan and lead new programs and workshops. We are currently looking for volunteers to facilitate groups and workshops about the following topics:
  • Gardening
  • Food & nutrition
  • Budgeting
  • Mental health supports and more!

Service Update

If you are a client at the SGBCHC, appointments with your doctor, nurse practitioner, or clinic nurse can be made by calling reception. In-person and virtual appointments are available with nurses and doctors, social work counselling, cognitive behavioural therapy through the Ontario Structured Psychotherapy Program, dietitian services, diabetes support and smoking cessation support services. Walk-in and scheduled appointments are available for addiction support services through the Rapid Access Addiction Medical (RAAM) Clinic.
